Sample Submission Guidelines

Collect & Seal

Use sterile containers. Ensure samples are representative of the batch and securely sealed to prevent cross-contamination.

Complete CoC Form

Fill out the digital form below, or download the physical PDF Chain of Custody form to include inside the shipping container.

Package Securely

Pack samples with appropriate refrigerant (ice packs/dry ice) to maintain temperature integrity during transit.

Ship to Laboratory

Ship via overnight courier. Ensure packages are clearly labeled with "Laboratory Samples - Keep Refrigerated" if applicable.
